Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents and patients
• Administer medications and treatments accurately and safely per facility protocols
• Monitor and record vital signs, changes in condition, and communicate findings to the care team
• Support daily living activities and promote dignity and independence for residents
• Collaborate with RNs, physicians, and interdisciplinary staff to coordinate care plans
• Maintain accurate documentation in the electronic medical record and complete required paperwork
• Follow infection control standards and facility policies to ensure a safe environment

Qualifications

• Current Licensed Practical Nurse (LPN) license in Pennsylvania or eligible to obtain one
• Basic Life Support (BLS) certification required; additional certifications a plus
• Comfortable administering medications and performing routine clinical procedures
• Strong communication skills and a team-player attitude
• Previous experience in long-term care, skilled nursing, or a related setting preferred

Education

• Graduate of an accredited practical nursing program
• Active LPN license in Pennsylvania
• Current BLS/CPR certification

• Medication administration and documentation best practices
• Wound care basics and dressing changes
• Vital sign assessment and trend recognition
• Infection prevention and control procedures
• Electronic medical record (EMR) navigation and charting
• Effective patient and family communication and bedside manner
