Roles & Responsibilities

• Provide compassionate, patient-centered nursing care as a Licensed Practical Nurse
• Administer medications and treatments accurately and document appropriately
• Monitor patient status and communicate changes to the interdisciplinary team
• Perform wound care, vital signs monitoring, and routine clinical procedures
• Assist with activities of daily living while promoting patient dignity and independence
• Collaborate with nurses, providers, and support staff to ensure continuity of care
• Maintain accurate, timely documentation in the patient record
• Support a positive, safe environment for patients, families, and coworkers

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing
• Basic Life Support (BLS/CPR) certification required or willing to obtain
• Clinical experience in a relevant setting is highly valued
• Strong communication skills and the ability to work as part of a multidisciplinary team
• Comfort with electronic health records and routine clinical documentation
• Immunizations and health screening per facility policy

Education

• Graduate of an accredited LPN/Vocational Nursing program
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and safe documentation practices
• Basic wound care and dressing changes
• Accurate vital signs assessment and patient monitoring
• Infection control and standard precautions
• Time management and prioritization in a busy clinical setting
• Patient education and family communication
