Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in a skilled setting
Administer medications and treatments accurately and on time
Observe and document patient condition, reporting significant changes to the care team
Collaborate with RNs, CNAs, therapists, and interdisciplinary staff to deliver coordinated care
Assist with daily living activities while promoting patient dignity and independence
Maintain accurate clinical records and ensure compliance with facility policies

Qualifications

Active LPN license in Pennsylvania or eligible to obtain licensure
Current Basic Life Support (BLS) / CPR certification or willingness to obtain
Comfortable working as part of a care team with strong communication skills
Demonstrated commitment to patient-centered care and professionalism
Ability to pass background checks and meet facility onboarding requirements

Education

LPN diploma or certificate from an accredited program
High school diploma or GED

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care and vital sign monitoring
Familiarity with electronic health records and charting
Strong patient assessment and clinical observation skills
Effective communication with residents, families, and clinical teams
