Roles & Responsibilities

Provide compassionate direct patient care that improves daily comfort and outcomes
Administer medications and treatments accurately and document thoroughly
Monitor patient status and promptly report changes to the clinical team
Assist with activities of daily living while promoting patient dignity and independence
Collaborate with nurses and interdisciplinary staff to deliver coordinated care
Maintain clear, timely charting and follow all facility policies and protocols
Support a positive environment for residents, families, and coworkers

Qualifications

Current LPN license in good standing (required)
CPR/BLS certification (or willingness to obtain upon hire)
Comfortable with medication administration, documentation, and basic assessments
Strong communication and teamwork skills to collaborate with interdisciplinary staff
Prior experience in long-term care or skilled nursing is a plus

Education

Practical Nursing diploma or equivalent from an accredited program
High school diploma or GED

Knowledge & Skills

Patient-centered care and basic clinical assessment
Medication administration and accurate charting
Infection control practices and safety protocols
Effective communication with patients, families, and clinical team
Time management and prioritization in a fast-paced setting
