Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in accordance with care plans
Administer medications and treatments accurately and document appropriately
Monitor and report changes in patient condition to the care team in a timely manner
Collaborate with interdisciplinary team members to deliver coordinated care
Assist with admissions, transfers and discharges while maintaining patient dignity
Maintain accurate clinical records and comply with facility policies and regulatory standards

Qualifications

Active LPN license (practical nurse) in good standing - required
Current BLS/CPR certification preferred or obtained upon hire as required by facility
Comfortable with medication administration, basic wound care and ADL assistance
Ability to pass required background checks and facility screenings
Strong communication skills and a team-oriented mindset

Knowledge & Skills

Clinical assessment and basic nursing procedures
Medication administration and documentation
Patient safety, infection control and standard precautions
Electronic charting or paper documentation practices
Effective communication with residents, families and interdisciplinary teams
Time management and prioritization during busy shifts
