Roles & Responsibilities

• Coordinate patient care from admission through discharge, ensuring safe transitions and continuity
• Develop and manage individualized plans of care in collaboration with the interdisciplinary team
• Lead discharge planning — arrange DME, SNF placements, and ambulance transport independently
• Perform utilization review using InterQual criteria and ensure regulatory compliance
• Document thoroughly and accurately in Epic and other charting systems
• Facilitate family meetings and contribute to treatment plan development
• Identify social determinants of health (homelessness, substance use, mental health needs) and connect patients with community resources
• Communicate effectively with HMOs, IPAs, and other payers to support care coordination

Qualifications

• Valid California state license (pending accepted but license must be active by start date)
• Minimum 2 years acute care hospital case management experience performing care coordination and discharge planning
• Travel experience required; first-time travelers are welcome (clinical screening applies)
• Proficiency with Epic and computerized documentation systems
• Working knowledge of InterQual criteria and utilization review processes
• Comfort working with a 1:20 patient ratio on day shifts and participating in weekend rotations
• Ability to organize and lead family meetings and participate in treatment plan development
• Competence arranging DME, SNF, and ambulance transport independently

Knowledge & Skills

• Care coordination and discharge planning in an acute hospital setting
• Utilization review and InterQual application
• Familiarity with CMS rules and DRG concepts
• Experience working with HMOs, IPAs, and payer processes
• Skilled in Epic and other charting systems
• Strong documentation and computerized documentation competency
• Ability to assess and support patients with mental health, substance abuse, and homelessness issues
• Family meeting facilitation and advance directives/POLST knowledge
• Knowledge of protective services processes and community resources
• Proficient in Microsoft Office applications
