Roles & Responsibilities

• Verify and clinically evaluate medication orders to ensure safe, evidence-based therapy.
• Prepare and compound sterile and non-sterile preparations, including TPN and chemo under strict sterile technique.
• Manage dosing for high-alert medications — anticoagulants, pediatric and renal dosing, and pharmacokinetic monitoring.
• Collaborate with physicians, nurses and interdisciplinary teams to optimize pharmacotherapy and patient outcomes.
• Oversee medication distribution systems (Pyxis) and maintain accurate inventory and override reporting.
• Perform medication reconciliation and therapeutic monitoring using Epic charting.
• Provide education and guidance to staff and patients on medication management and safety.

Qualifications

• Active RPh license in the state of California (State licensed required)
• PharmD or equivalent (PharmD listed as preferred/required credential)
• Minimum 2 years of relevant pharmacy experience (inpatient/acute care preferred)
• Travel/contract experience preferred — this role requires travel experience
• Certifications preferred: BCPS and BLS; other relevant certifications welcomed
• Demonstrated experience with compounding, chemotherapy handling, IV preparation, TPN and sterile technique

Education

• PharmD
• Active RPh (Pharmacist) licensure

Knowledge & Skills

• Strong clinical dosing skills: anticoagulants, cardiac, pediatric, geriatric, renal and pharmacokinetic dosing
• Experience with chemotherapy preparation, packaging antineoplastics and high-alert medication management
• Competent in sterile compounding, IV piggyback preparation and mixers for TPN
• Familiarity with Epic electronic charting and Pyxis medication dispensing systems
• Inventory management and reporting skills (inventory reports, override reports)
• Excellent communication and interdisciplinary collaboration skills
