Roles & Responsibilities

• Perform thorough speech-language evaluations to assess patient needs and create individualized plans
• Deliver daily treatments that make measurable progress in communication and swallowing function
• Coordinate and manage timely discharges, ensuring safe transitions and clear caregiver education
• Collaborate closely with the therapy team to provide cohesive, patient-centered care at Odelia Healthcare
• Document interventions and progress according to CRG policies and facility standards
• Meet productivity expectations and manage caseload efficiently to support facility goals

Qualifications

• Licensed Speech-Language Pathologist (SLP) or equivalent credential appropriate for practice
• 1 to 3 years clinical experience (per job posting)
• Prior SNF experience strongly preferred
• Ability to work on-site in black scrubs and follow CRG policies & procedures
• Comfortable working daytime schedules and meeting productivity requirements (90%)
• Willingness to be present in the building to receive guaranteed hours and to provide excellent patient care

• Clinical expertise in evaluations, daily treatments, and discharge planning
• Experience in skilled nursing facility practices and patient-centered rehab
• Strong documentation skills and familiarity with facility policies
