Roles & Responsibilities

• Perform thorough physical therapy evaluations and create individualized plans of care
• Deliver skilled rehab treatments in patient rooms, hallways, outdoor areas, and the rehab gym
• Collaborate with the interdisciplinary rehab team to report progress, equipment needs, and discharge plans
• Participate in rounds and care conferences to support coordinated patient care
• Document all evaluations, interventions and progress accurately in Cerner and NetHealth
• Provide safe, evidence-based interventions across acute, swing bed, nursing home and outpatient settings
• Recommend and coordinate necessary adaptive equipment and discharge resources

Qualifications

• Active NC Physical Therapy license (required)
• Current CPR certification (required)
• Minimum 2 years of clinical experience (listed); must have experience in hospital and/or nursing home settings
• Preferred: 3+ years in a medical setting (rehab-focused experience highly valued)
• Comfortable floating between acute, nursing home and outpatient settings
• Proven documentation skills in electronic health records (Cerner and/or NetHealth experience a plus)
• Strong communication and teamwork skills

Knowledge & Skills

• Skilled in physical therapy evaluations and establishing effective plans of care
• Experience delivering rehab services in inpatient, nursing home and outpatient environments
• Familiarity with Cerner and NetHealth documentation systems
• Knowledge of common rehab equipment and safe patient handling
• Ability to assess discharge needs and coordinate with the care team
• Comfort treating patients in diverse settings (room, gym, outdoor areas)
