Roles & Responsibilities
- Provide compassionate direct and indirect patient care under RN or LPN direction
- Assist residents with activities of daily living including bathing, dressing, mobility, and toileting
- Deliver personalized resident care to enhance comfort, dignity, and quality of life
- Maintain a safe, clean, and comfortable environment for all residents
- Collaborate closely with nursing and interprofessional teams to support resident care plans
- Accurately document care, observations, and any changes in resident condition
- Support infection control practices and follow facility policies to protect residents and staff
Qualifications
- 2+ years of Long Term Care CNA experience required
- Active Illinois CNA license (must be current and active)
- Current American Heart Association BLS certification required
- Completed work history and skills checklist current within 1 year (Long Term Care)
- Two supervisory reference forms with dates employed (required)
- Candidates must provide permanent address; applicants within 50 mile radius of facility are not eligible
- Applicants who were direct employees of the facility within the last 2 years are not eligible
Education
- Active Illinois CNA license (required)
- BLS (AHA) certification (required)
Knowledge & Skills
- Strong experience with ADL assistance and resident personal care
- Ability to maintain resident safety and a clean environment
- Comfortable communicating with residents, families, and clinical staff
- Basic charting and documentation skills
- Awareness of infection control and patient safety practices
- Reliable, compassionate, and team-oriented approach to care
