Roles & Responsibilities

• Provide compassionate direct and indirect patient care under RN or LPN direction
• Assist residents with activities of daily living including bathing, dressing, mobility, and toileting
• Deliver personalized resident care to enhance comfort, dignity, and quality of life
• Maintain a safe, clean, and comfortable environment for all residents
• Collaborate closely with nursing and interprofessional teams to support resident care plans
• Accurately document care, observations, and any changes in resident condition
• Support infection control practices and follow facility policies to protect residents and staff

Qualifications

• 2+ years of Long Term Care CNA experience required
• Active Illinois CNA license (must be current and active)
• Current American Heart Association BLS certification required
• Completed work history and skills checklist current within 1 year (Long Term Care)
• Two supervisory reference forms with dates employed (required)
• Candidates must provide permanent address; applicants within 50 mile radius of facility are not eligible
• Applicants who were direct employees of the facility within the last 2 years are not eligible

Education

• Active Illinois CNA license (required)
• BLS (AHA) certification (required)

Knowledge & Skills

• Strong experience with ADL assistance and resident personal care
• Ability to maintain resident safety and a clean environment
• Comfortable communicating with residents, families, and clinical staff
• Basic charting and documentation skills
• Awareness of infection control and patient safety practices
• Reliable, compassionate, and team-oriented approach to care
