Roles & Responsibilities
- Provide skilled home health nursing care during in-home visits, delivering compassionate, patient-centered services
- Administer medications and perform medication assessment/management across multiple routes (oral, IM/SQ, ear/eye, NG/G-tube)
- Perform wound care and Wound Vac management with attention to documentation and healing goals
- Document thoroughly and accurately in EPIC (remote), Rover (mobile) and OASIS to ensure continuity of care
- Implement and model infection control precautions to keep patients and communities safe
- Educate patients and caregivers on disease management, ostomy/catheter care, wound care and safe home practices
- Coordinate care with the interdisciplinary team and communicate changes in condition promptly
- Manage your own schedule and travel between patient homes — reliable transportation required
- Meet productivity expectations (may visit up to 6 patients per day; 30 productivity points standard)
Qualifications
- Active LPN license in Oregon — pending licenses considered (pending license accepted)
- Minimum 1 year of skilled Home Health experience required
- BCLS (BLS) certification required
- Comfortable and experienced with EPIC, Rover and OASIS charting systems
- Able to pass facility immunization requirements: primary COVID series plus one booster
- Reliable personal transportation and willingness to travel up to roughly 60 miles total drive time on occasional days
- Able to complete a 40-hour orientation and integrate into home health workflows
Education
- Licensed Practical Nurse (LPN) diploma/certificate or equivalent program completion
Knowledge & Skills
- Medication administration and management (oral, IM, SQ, ear/eye drops, NG/G-tube)
- Wound care, including wound vac experience
- Ostomy care and catheter care education
- Disease management principles for adults and geriatric patients
- Infection control precautions and safe home-based care practices
- Proficiency with EPIC, Rover and OASIS charting
- Strong patient education, communication and care coordination skills
- Comfort working independently in patients' homes and managing time effectively
