Roles & Responsibilities
- Conduct thorough biopsychosocial assessments and use findings to develop individualized, person-centered plans of care
- Coordinate care across internal and external team members to ensure seamless service delivery
- Provide meaningful patient and caregiver education to support safe transitions and optimal outcomes
- Document clearly and promptly in EPIC, Oasis, and Rover to reflect care delivery, goals, and patient response
- Participate in care conferences and communicate key information to facilitate collaborative decision-making
- Manage case assignments efficiently (may visit up to 6 patients/day; maintain expected productivity)
- Assist with discharge planning and connect patients to community resources such as Medicaid, HCBS, Medicare waivers, and Veterans programs
- Apply protective services knowledge (adult protective services, guardianship, power of attorney) when patient safety concerns arise
Qualifications
- Licensed Master Social Worker (LMSW) with BOTH Oregon (OR) and Washington (WA) licensure as required by the facility (pending status accepted with confirmation)
- Minimum 1 year of skilled home health experience required (first-timers may be considered in some cases)
- Current BCLS certification required
- Experience with OASIS and EPIC charting (EPIC Remote Client and Rover app preferred)
- Reliable personal transportation and ability to travel daily to patient homes (occasional ~60-mile radius)
- Facility requires primary series COVID vaccination plus one booster
- Strong documentation, assessment, care coordination, and discharge-planning skills
Education
- Master of Social Work (MSW) — required for LMSW licensure / or equivalent degree necessary for LMSW
Knowledge & Skills
- Case management and care coordination across interdisciplinary teams
- Biopsychosocial assessment and development/revision of person-centered care plans
- OASIS assessment and EPIC charting proficiency
- Patient and caregiver education and effective communication
- Familiarity with community resources: Agency on Aging, HCBS, Medicare waiver, Medicaid, Veterans Choice Program
- Knowledge of protective services (adult protective services, guardianship, power of attorney)
- Ability to document per coverage criteria and produce clear discharge summaries
- Awareness of cultural issues, ADA, advance directives, and high-risk intervention considerations
