Shift Type: Night shift, 11:00 PM - 7:30 AM NOC — specific days not specified (schedule varies)

Roles & Responsibilities
- Provide compassionate bedside care to residents and respond to their clinical needs promptly
- Administer medications and treatments accurately and document per facility policy
- Monitor patient conditions, record observations, and report changes to the care team
- Collaborate with RNs, physicians, and multidisciplinary staff to develop and follow care plans
- Support safe admissions, discharges, and transfers while maintaining clear documentation
- Educate residents and families about care instructions and answer questions with empathy
- Follow infection control, safety protocols, and facility policies to protect residents and staff
Qualifications
- Active Licensed Practical Nurse (LPN) license (Pennsylvania licensure required or eligible)
- Current BLS/CPR certification (or willingness to obtain upon hire)
- Proof of required immunizations and ability to pass background/health screenings
- Professional, patient-centered attitude with strong communication and teamwork skills
- Previous LPN experience in long-term care, skilled nursing, or similar setting preferred
Education
- Graduate of an accredited practical nursing program or equivalent LPN training
- Active LPN certification/licensure required
Knowledge & Skills
- Strong clinical assessment and basic nursing skills (vitals, wound care, ADL support)
- Medication administration and accurate charting/documentation
- Familiarity with infection control standards and safety practices
- Effective verbal and written communication with residents, families, and staff
- Time management and ability to prioritize in a fast-paced environment
- Team-oriented mindset and willingness to support colleagues across shifts
