Roles & Responsibilities

• Provide compassionate bedside care to residents and respond to their clinical needs promptly
• Administer medications and treatments accurately and document per facility policy
• Monitor patient conditions, record observations, and report changes to the care team
• Collaborate with RNs, physicians, and multidisciplinary staff to develop and follow care plans
• Support safe admissions, discharges, and transfers while maintaining clear documentation
• Educate residents and families about care instructions and answer questions with empathy
• Follow infection control, safety protocols, and facility policies to protect residents and staff

Qualifications

• Active Licensed Practical Nurse (LPN) license (Pennsylvania licensure required or eligible)
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Proof of required immunizations and ability to pass background/health screenings
• Professional, patient-centered attitude with strong communication and teamwork skills
• Previous LPN experience in long-term care, skilled nursing, or similar setting preferred

Education

• Graduate of an accredited practical nursing program or equivalent LPN training
• Active LPN certification/licensure required

Knowledge & Skills

• Strong clinical assessment and basic nursing skills (vitals, wound care, ADL support)
• Medication administration and accurate charting/documentation
• Familiarity with infection control standards and safety practices
• Effective verbal and written communication with residents, families, and staff
• Time management and ability to prioritize in a fast-paced environment
• Team-oriented mindset and willingness to support colleagues across shifts
