Roles & Responsibilities

• Assess patients' communication, language, cognitive-communication and swallowing needs using evidence-based tools
• Develop individualized treatment plans with clear, measurable goals
• Implement therapeutic interventions and adapt techniques to each patient's progress
• Collaborate with nurses, therapists, physicians and families to coordinate comprehensive care
• Document assessments, plans and progress in the facility’s required format in a timely manner
• Educate patients, families and staff on communication strategies and safe swallowing practices
• Participate in team meetings and contribute to a culture of continuous improvement

Qualifications

• Active state license as a Speech-Language Pathologist (required)
• ASHA CCC-SLP preferred or equivalent certification/credentials
• Experience in rehabilitation, contract, or long-term care settings is a plus
• Current CPR/BLS per facility requirements (if applicable)
• Clean background check and ability to meet facility onboarding requirements

Education

• Master's degree in Speech-Language Pathology (or equivalent clinical graduate degree)

Knowledge & Skills

• Proven skills in speech, language, cognition and dysphagia assessment and intervention
• Familiarity with documentation standards and electronic health records
• Strong communication and interdisciplinary collaboration skills
• Ability to prioritize caseload and manage time effectively
• Comfort working with a diverse patient population and adapting therapy to individual needs
