Roles & Responsibilities
- Assess and monitor cardiac rehabilitation patients before, during, and after exercise sessions to ensure safety and progress
- Develop and implement individualized cardiac rehab care plans in collaboration with physicians and therapists
- Lead supervised exercise sessions, monitor vitals and ECG changes, and respond immediately to clinical changes
- Provide patient and family education on heart-healthy lifestyles, medication adherence, and home exercise plans
- Document patient encounters, progress notes, and outcomes accurately in the electronic medical record
- Participate in Trinity Specialty Specific Competency Testing and maintain required certifications
- Coordinate care with the cardiology team, referring providers, and ancillary services to optimize patient recovery
Qualifications
- Active Registered Nurse (RN) license (New York) or compact license as applicable
- BLS (Basic Life Support) required
- ACLS (Advanced Cardiac Life Support) required
- Cardiology or cardiac rehabilitation clinical experience required
- Successful completion of Trinity Skills Checklist and Trinity Specialty Specific Competency Testing
- Ability to pass license/certificate verification and standard 10-panel drug screen
- Up-to-date immunizations and health screening: COVID-19 vaccine, MMR, Hep B, Influenza, 2-step PPD or IGRA, Varicella, fit test, and physical/health statement
Education
- Graduate of an accredited nursing program (Associate Degree in Nursing or BSN preferred)
Knowledge & Skills
- Strong understanding of cardiac rehabilitation principles and exercise prescription
- Comfortable reading telemetry/ECG rhythm strips and recognizing arrhythmias
- Knowledge of cardiovascular medications and hemodynamic monitoring
- Excellent patient education and motivational interviewing skills
- Strong clinical assessment, critical thinking, and emergency response abilities
- Clear communication and teamwork skills for working with cardiology providers and rehab staff
- Proficiency with electronic medical records and accurate clinical documentation
