Roles & Responsibilities

• Develop collaborative, patient-centered care plans with physicians, patients, and caregivers
• Conduct ongoing assessments of clinical, functional, and emotional status to track progress
• Coordinate interdisciplinary care and lead communication across the care team
• Document evaluations, treatment goals, and recertifications accurately and timely
• Educate patients and caregivers on self-care techniques, medications, and diet as ordered
• Notify physicians and VNA colleagues promptly about significant changes in patient condition
• Participate in case conferences and discharge planning to ensure smooth transitions

Qualifications

• Licensed Registered Nurse in Vermont and New Hampshire OR an unencumbered Nurse Licensure Compact (NLC) license
• Graduate of an accredited school of nursing
• Minimum 2 years of Home Health and Hospice experience
• Ability to work day shift schedule with rotating weekend coverage

Education

• Graduate of an accredited school of nursing

• Care planning and patient assessment expertise
• Strong care coordination and interdisciplinary communication
• Accurate clinical documentation and charting
• Patient and caregiver education and health counseling
• Experience with hospice and home health best practices
• Medication and diet instruction knowledge
