Roles & Responsibilities

• Coordinate patient care across teams to ensure safe, timely discharges
• Perform utilization review using InterQual and Milliman guidelines to support appropriate care
• Manage case loads with attention to admission criteria, DRG and coding implications
• Communicate clearly with patients, families and multidisciplinary teams to advocate for patient needs
• Document thoroughly in the charting/EHR system to maintain regulatory compliance and continuity of care
• Problem-solve creatively in a high-volume environment to improve patient flow and outcomes
• Float to other UCSF facilities when needed to support staffing and patient care demands

Qualifications

• Current CA RN license required
• AHA BLS required
• Minimum 2 years of inpatient/hospital nursing experience
• COVID-19 vaccine plus booster required (medical/religious exemptions only)
• Demonstrated strong written and oral communication, organization and analytic skills
• Self-directed, assertive and creative in systems planning and patient care management
• Master's degree preferred

Knowledge & Skills

• Care coordination and discharge planning expertise
• Case management and utilization review processes
• Familiarity with InterQual and Milliman criteria
• Knowledge of ICD-10, CPT coding and DRG concepts
• Understanding of HEDIS, NCQA, CMS and Joint Commission standards
• HIPAA, OSHA and worker's compensation regulatory knowledge
• Comfort using charting/EHR systems in an acute hospital setting
