Roles & Responsibilities

• Provide skilled physical therapy evaluations and treatments for residents in a Skilled Nursing Facility, focusing on mobility, balance, and functional independence
• Develop individualized, measurable treatment plans and adjust them as residents progress
• Collaborate with nursing and interdisciplinary teams to coordinate care and optimize outcomes
• Document accurately and timely in the medical record to support care continuity and regulatory compliance
• Educate residents and families on safe mobility, fall prevention, and home exercise programs
• Participate in care planning meetings and contribute clinical expertise to discharge planning

Qualifications

• Active Physical Therapist license in Michigan (or compact licensure where applicable)
• Experience or comfort working in a Skilled Nursing Facility or with geriatric populations
• Current BLS (Basic Life Support) certification
• Strong clinical reasoning, documentation skills, and ability to work collaboratively with a care team
• Professional, reliable, and flexible approach to meet patient and facility needs

Education

• Doctor of Physical Therapy (DPT) preferred or Bachelor/Master in Physical Therapy with equivalent licensure

Knowledge & Skills

• Skilled in therapeutic exercise, gait training, transfers, and balance interventions
• Knowledge of SNF clinical workflows, Medicare documentation, and FIM-style functional assessment
• Excellent patient and family education skills to promote adherence and safe transitions
• Good communication and teamwork skills to collaborate with nursing, OT, SLP, and case managers
• Comfortable using electronic medical records for evaluation and progress notes
