Roles & Responsibilities

• Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders using evidence-based practice
• Develop, implement, and adjust individualized plans of care and treatment goals
• Administer and interpret standardized assessments to guide clinical interventions
• Document all regulatory and clinical information in the electronic medical record, including accurate recording of treatment minutes
• Prepare comprehensive discharge summaries that clearly reflect services provided and outcomes
• Communicate patient progress, barriers, and treatment plans with supervisors and interdisciplinary team members
• Provide counseling and education to patients, families, caregivers, and other providers
• Obtain necessary DME/supplies and make appropriate referrals based on patient outcomes
• Participate in required meetings (patient care conferences, utilization reviews, rehabilitation meetings) and facility trainings

Qualifications

• Active Texas state license (required) and adherence to state practice acts
• CCC (ASHA Certificate of Clinical Competence) required — please submit documentation
• Active ASHA membership preferred/expected
• Minimum 1 year of SLP experience (required); LTC or rehab experience required
• New graduates accepted if they have Level II experience (not Level I observation only)
• Experience supervising CFY or student SLPs and willingness to mentor
• Comfortable with electronic documentation/EMR and accurate minute recording
• Ability to participate in interdisciplinary meetings and facility trainings
• Willingness to provide one reference at time of submission
