Roles & Responsibilities

• Perform thorough physical therapy evaluations and develop individualized plans of care
• Provide hands-on treatment across settings including patient rooms, hallways, outdoor areas, and the rehab gym
• Document care accurately and promptly using Cerner and NetHealth
• Participate in patient rounding and interdisciplinary care conferences to drive coordinated discharge planning
• Communicate frequently with the rehab team about patient progress, equipment needs, and transitions of care
• Support patient and family education to maximize recovery and safe discharge
• Assist with equipment assessment and ensure safe use during treatment sessions

Qualifications

• NC Physical Therapy license required
• Current CPR certification required
• Minimum 2 years of experience (as listed); preference for clinicians with 3+ years in a medical setting
• Experience in hospital and/or nursing home settings is a must
• Comfortable working in acute, swing, nursing home, and outpatient environments and willing to float
• Familiarity with Cerner and NetHealth documentation systems preferred

Knowledge & Skills

• Skilled in PT evaluations, clinical reasoning, and plan of care development
• Strong documentation skills using Cerner and NetHealth
• Ability to collaborate with interdisciplinary teams and participate in care conferences
• Knowledge of typical rehab equipment and how to deliver treatments in multiple environments
• Good patient-handling techniques and focus on safety and outcomes
• Effective communication skills with patients, families, and clinical teams
