Roles & Responsibilities

Provide compassionate direct patient care including vital signs, hygiene assistance, and daily observations
Administer medications and follow care plans accurately and safely
Document patient status and interventions clearly in the electronic medical record
Collaborate with RNs, providers, and interdisciplinary team members to support patient outcomes
Monitor patients for changes and escalate concerns promptly to ensure safety
Support admissions, discharges, and transitions with professionalism and care

Qualifications

Valid LPN license in Pennsylvania (required)
Graduate of an accredited practical nursing program or equivalent diploma
Current BLS/CPR certification or willingness to obtain upon hire
Ability to pass background check and meet facility immunization requirements
Strong communication skills and a patient-centered attitude
Previous LPN experience in a clinical setting is a plus

Education

Diploma or certificate from an accredited practical nursing program (LPN/LVN)

Knowledge & Skills

Medication administration and basic IV/SC familiarity (as applicable)
Patient assessment and monitoring (vitals, reporting trends)
Accurate clinical documentation and charting in EMR systems
Infection control and standard precautions
Prioritization, time management, and teamwork in a busy healthcare setting
Compassionate bedside manner and effective communication with patients and families
