Roles & Responsibilities

Provide compassionate direct patient care, ensuring dignity and comfort
Administer medications and treatments accurately and safely
Monitor patient conditions and communicate changes to the care team
Document care clearly and timely in the patient chart
Assist with activities of daily living and mobility as needed
Collaborate with nurses and interdisciplinary staff to deliver excellent care

Qualifications

Active LPN license in Pennsylvania (or compact/reciprocal where applicable)
Current BLS/CPR certification or willingness to obtain promptly
Demonstrated clinical skills in medication administration, wound care, and basic assessments
Strong communication and teamwork mindset—patients and coworkers appreciate it
Ability to pass background and health screenings per facility policy

Education

Completion of an accredited practical nursing program (LPN/LVN diploma or certificate)

Knowledge & Skills

Solid knowledge of basic nursing principles and infection control
Proficiency with medication administration and documentation
Comfortable with electronic charting or willingness to learn
Good assessment skills and ability to escalate concerns appropriately
Strong interpersonal skills and a person-centered approach to care
