Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in a long-term care setting
Administer medications accurately and document per facility policy
Perform routine assessments and communicate changes in patient status to the care team
Maintain accurate charts and complete required documentation in a timely manner
Collaborate with nurses, therapists, and support staff to deliver person-centered care
Assist with activities of daily living while promoting dignity and independence
Follow infection control protocols and maintain a safe environment for residents

Qualifications

Active, unencumbered LPN license for the state of Pennsylvania
Graduate of an accredited practical/vocational nursing program
Current CPR/BLS certification (or obtain upon hire)
Ability to pass background check and meet facility immunization requirements
Strong communication skills and a team-focused attitude
Previous long-term care experience preferred but new grads welcome to apply

Education

Vocational/Practical Nursing diploma or equivalent required; active LPN license in PA required

Knowledge & Skills

Medication administration and documentation best practices
Basic nursing assessments and resident monitoring
Charting in electronic or paper medical records
Infection prevention and control procedures
Strong interpersonal skills for working with residents and families
Time management and prioritization in a fast-paced care setting
