Roles & Responsibilities

• Provide compassionate, direct nursing care to assigned residents and patients
• Administer medications and treatments safely and accurately according to policy
• Monitor and record vital signs, behavior, and changes in patient condition
• Document care and interventions clearly and timely in the medical record
• Collaborate with RNs, CNAs, and interdisciplinary team members to support care plans
• Educate patients and families on basic care, medication, and safety
• Follow infection control practices and promote a safe care environment
• Escalate concerns promptly to ensure excellent patient outcomes

Qualifications

• Active LPN license in Pennsylvania (required)
• Current BLS/CPR certification or willingness to obtain
• Completion of an accredited practical nursing program
• Comfort working flexible per diem schedules and responding to facility needs
• Ability to pass background check and drug screening as required
• Strong communication skills and a team-oriented mindset

Education

• Diploma or certificate from an accredited Practical Nursing (LPN) program

Knowledge & Skills

• Medication administration and basic clinical nursing skills
• Resident/patient assessment and documentation best practices
• Basic wound care and infection prevention principles
• Familiarity with electronic medical records or clinical charting
• Time management and prioritization in a fast-paced care setting
• Clear communication with patients, families, and the care team
