Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients
- Administer medications and treatments safely and accurately
- Monitor and record vital signs, reporting changes promptly
- Collaborate with RNs and the care team to create and follow care plans
- Document assessments and care in the medical record with clarity
- Educate patients and families about care, medications, and follow-up
Qualifications
- Active LPN license in Pennsylvania (required)
- Current CPR/BLS certification (required or obtained upon hire)
- Comfortable working in long-term care/rehabilitation settings
- Strong communication skills and a team-focused attitude
- Previous clinical experience preferred and appreciated
Education
- Practical Nursing diploma or LPN certificate (or equivalent)
Knowledge & Skills
- Medication administration and charting best practices
- Basic wound care and infection control procedures
- Patient assessment and vital sign monitoring
- Familiarity with electronic medical records and documentation
- Excellent interpersonal and teamwork skills
