Roles & Responsibilities

• Provide compassionate, skilled nursing care to residents consistent with LPN scope of practice
• Administer medications and treatments accurately and document in the patient record
• Perform focused assessments, monitor patient status and report changes promptly
• Collaborate with nurses, therapists and interdisciplinary team members to support care plans
• Maintain clear, timely charting and ensure regulatory and facility documentation standards are met
• Support infection control practices and promote a safe, respectful care environment

Qualifications

• Valid Licensed Practical Nurse (LPN) license in New Mexico or eligible for licensure
• Current Basic Life Support (BLS) certification preferred or willing to obtain
• Comfortable administering medications, performing assessments and documenting care
• Strong communication skills and ability to work as part of a multidisciplinary team
• Professional attitude, reliability, and commitment to resident-centered care

Education

• Completion of an accredited Practical Nursing program (LPN/LVN diploma or certificate)
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and monitoring
• Basic patient assessment and vital sign interpretation
• Wound care basics and pressure injury prevention
• Electronic health record (EHR) charting and documentation
• Infection control procedures and safe clinical practices
• Time management, prioritization and teamwork
