Roles & Responsibilities

• Provide compassionate direct care to residents, assisting with activities of daily living such as bathing, dressing, grooming, and toileting
• Monitor and record vital signs and resident condition, escalating changes to nursing staff promptly
• Support safe mobility and transfers, using proper body mechanics and assistive devices
• Document care accurately and timely in the resident record to ensure continuity of care
• Follow infection control and safety protocols to protect residents and team members
• Collaborate with interdisciplinary team members to promote resident dignity, comfort, and quality of life

Qualifications

• Current Licensed Nursing Assistant (LNA) certification; T-LNA or equivalent accepted
• Licensure valid in VT or NH as applicable for assignment
• Basic Life Support (BLS/CPR) certification preferred or willing to obtain
• Up-to-date immunizations and TB screening as required by facility
• Ability to pass background check and drug screening per facility policy
• Strong communication skills and a proven ability to work as part of a care team

Education

• State-approved LNA/CNA program completion or equivalent
• High school diploma or GED preferred

Knowledge & Skills

• Knowledge of resident-centered care and ADL assistance best practices
• Skilled in taking and documenting vital signs and observations
• Familiarity with infection prevention and safe patient handling
• Good time management and ability to prioritize tasks on a busy floor
• Empathy, patience, and strong interpersonal skills for working with elders
• Basic charting and documentation skills
