Shift Type: Monday-Friday, 8:00 AM - 5:00 PM + occasional weekends and holidays

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver compassionate home health care to adult patients across their homes
Perform start of care visits, recertifications, and daily visits with precision
Case manage approximately 30 patients using a 6.5 points per day system
Collaborate with clinical managers and educators for optimal patient outcomes
Chart efficiently using Homecare Homebase (HCHB) system

Qualifications

Active RN license in Kentucky
BLS Certification required
1 year of MedSurg floor experience in a hospital setting
Proficiency in HCHB (Homecare Homebase) - a must-have skill

Education

RN licensure; BSN not required

Knowledge & Skills

Expertise in home health nursing for adults
Strong case management abilities
Familiarity with point-based visit systems
Comfortable with SOC, recerts, and routine daily visits
