Roles & Responsibilities

Deliver impactful education and case management to patients with diabetes, prediabetes, and gestational diabetes
Maintain vital diabetes registries to ensure top-tier patient tracking and care
Coordinate annual exams, lab tests, immunizations, and education per IHS Standards
Drive primary and secondary prevention goals alongside the SEARHC Diabetes team
Facilitate training on IHS Standards for providers, dietitians, nurses, and staff
Utilize audit data and tools to elevate quality of care
Support SEARHC Seven Standards of Excellence while assisting staff, visitors, and patients with warmth

Qualifications

Valid current RN license in Alaska
Certified Diabetes Educator or eligible to certify within 2 years
Clinical Competency Assessment within 3 months of hire and every 3 years
3 years general nursing or dietetic experience preferred
1 year in diabetic care and management preferred

Education

Registered Nurse (RN) license required

Knowledge & Skills

Expertise in diabetes case management practices and procedures
Strong health promotion and patient education skills
Knowledge of Alaska Native customs and cultural sensitivities
Skilled in diabetic care procedures tailored to patient needs
Tactful communication in high-stress situations
Proficient oral, written, and computer skills
Exceptional teaching and organizational abilities
