Roles & Responsibilities

• Provide compassionate personal care and assistance with activities of daily living (bathing, dressing, grooming)
• Assist residents with mobility and transfers using safe body mechanics
• Take and record vital signs and report changes in condition promptly
• Complete required documentation and maintain accurate records
• Follow infection control practices and facility policies to keep residents safe
• Participate in Trinity Specialty Competency Testing and Trinity Skills Checklist to maintain high standards
• Collaborate with the care team and communicate effectively with residents and families

Qualifications

• Current Certified Nursing Assistant (CNA) credential
• BLS (Basic Life Support) certification
• Willingness to complete license/certificate verification
• Ability to pass Trinity 10-panel drug screen and required health screenings
• Up-to-date immunizations per policy (MMR, Hepatitis B, Varicella, Tdap, COVID-19, Influenza)
• Completion of 2-step PPD or IGRA (Quantiferon) as required
• Reliable, compassionate, and able to work independently in a per diem pool

Knowledge & Skills

• Skilled in assisting with ADLs and resident personal care
• Knowledge of infection control and safe patient handling
• Comfortable taking vital signs and observing for clinical changes
• Good communication and teamwork skills
• Ability to follow care plans and facility protocols
• Detail-oriented with strong documentation habits
