Roles & Responsibilities

• Provide compassionate direct patient care and assist with activities of daily living
• Support safe transfers and mobility, using proper body mechanics and lifting techniques
• Monitor and record vital signs and report changes in patient condition promptly
• Help maintain a clean, comfortable environment and follow infection control practices
• Communicate clearly and collaboratively with nurses, therapists, patients, and families
• Document care accurately and timely in the patient chart to support continuity

Qualifications

• Current Certified Nursing Assistant (CNA) credential; T-CNA where applicable
• Basic Life Support (BLS) certification preferred or willing to obtain
• Up-to-date immunizations and ability to pass required background checks
• Comfortable working 12+ hour day shifts and able to handle physical demands of patient care
• Previous experience in long-term care or rehab settings is a plus
• Strong communication skills and a patient-centered attitude

Education

• High school diploma or equivalent
• Completion of an accredited CNA training program and current CNA certification

Knowledge & Skills

• Knowledge of activities of daily living and basic nursing care
• Skill in taking and recording vital signs accurately
• Understanding of infection prevention, safety protocols, and patient handling
• Comfortable with electronic charting and documentation best practices
• Ability to work as part of a multidisciplinary care team and communicate effectively
