Roles & Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans that promote independence and functional gains
• Deliver targeted therapy interventions to improve mobility, strength, balance, and safety
• Complete thorough evaluations, progress notes, and discharge documentation in line with facility and regulatory standards
• Collaborate with nurses, therapists, and the interdisciplinary team to coordinate excellent patient care
• Educate patients and families about therapy goals, safety precautions, and practical home exercise plans
• Adhere to facility, state, and federal guidelines to ensure safe, compliant practice

Qualifications

• Active California Physical Therapist license in good standing
• Graduate of an accredited Physical Therapy program
• Skilled Nursing Facility or rehabilitation experience preferred
• Strong communication, documentation, and clinical decision-making skills
• Comfortable working independently and as part of a multidisciplinary team

Education

• Graduate of an accredited Physical Therapy program

Knowledge & Skills

• Post-acute rehabilitation assessment and treatment planning
• Therapeutic exercise, gait and balance training, and mobility interventions
• Skilled clinical documentation and regulatory compliance
• Patient and family education with clear, compassionate communication
• Interdisciplinary collaboration and care coordination
• Sound clinical decision-making and prioritization in a busy SNF environment
