Roles & Responsibilities

• Provide skilled physical therapy services to residents in a skilled nursing facility setting
• Perform thorough patient assessments and develop individualized rehabilitation plans
• Deliver targeted interventions to improve mobility, balance, strength, and functional independence
• Collaborate with nursing and interdisciplinary team members to coordinate patient care
• Document clinical progress and discharge planning clearly and professionally
• Educate residents and families on home programs and safe mobility techniques

Qualifications

• PT or DPT (listed as primary skills in the job posting)
• Open to all experience levels — any, including New Graduates (posting explicitly welcomes new grads)
• Comfort working in a Skilled Nursing Facility (SNF) practice setting

Education

• DPT or appropriate PT credential (DPT listed among primary skills)

Knowledge & Skills

• Clinical assessment and therapeutic intervention skills for adult rehabilitation
• Experience or comfort with skilled nursing facility workflows and documentation
• Strong interpersonal and communication skills for resident- and team-focused care
• Ability to create practical, functional treatment plans that drive outcomes
