Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with on-call rotation for evenings, weekends, and holidays

Hours per Week: 40

Candidate Pay Rate: $62 per hour

Roles & Responsibilities

Conduct initial assessments of clients and families to determine home health needs
Develop and implement plans of care, including setting goals and incorporating therapeutic actions
Administer medications and treatments as prescribed by physicians
Provide health care instructions to clients and families
Coordinate client care and communicate with physicians

Qualifications

Current RN license
BLS certification required
Experience in Home Health and Hospice care
Valid Driver's License and Auto Insurance

Education

RN degree from an accredited institution
