Roles & Responsibilities
- Evaluate residents and perform thorough physical therapy assessments
- Develop individualized treatment plans focused on functional outcomes
- Deliver skilled therapeutic interventions and progress patients toward goals
- Document interventions and progress clearly and timely in the chart
- Collaborate with nursing and rehab team members to coordinate care
- Educate residents and families on home exercise programs and safety
- Participate in discharge planning and transitions of care
Qualifications
- PT or DPT credential noted as core skills for this role
- Open to any experience level, including new graduates
- Ability to float occasionally as requested
- Professional communication skills and a patient-centered approach
Knowledge & Skills
- Strong knowledge of physical therapy assessment and treatment principles
- Experience or comfort working in a skilled nursing facility / geriatric setting
- Ability to create functional, goal-oriented treatment plans
- Good documentation practices and teamwork with interdisciplinary staff
- Clinical reasoning and safe patient-handling skills
