Roles & Responsibilities
- Provide compassionate basic nursing care and daily support to residents
- Administer medications and treatments accurately and safely
- Monitor residents’ conditions and promptly report changes to the care team
- Document resident care, observations and communications clearly and timely
- Collaborate with nurses and interdisciplinary staff to promote excellent outcomes
- Support resident comfort, dignity and participation in daily activities
Qualifications
- Active LPN license (Pennsylvania) required
- Current CPR/BLS certification preferred or willing to obtain
- Clear background check and ability to pass required screenings
- Strong communication and team collaboration skills
- Compassionate patient-focused attitude and reliable work ethic
Knowledge & Skills
- Medication administration and basic clinical assessments (vital signs, intake/output)
- Basic wound care and infection control practices
- Accurate clinical documentation and charting habits
- Ability to work effectively in a long-term care or skilled nursing environment
- Interpersonal skills for working with residents, families and the care team
