Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as a Licensed Practical Nurse
• Administer medications and treatments accurately and document care in a timely manner
• Monitor residents’ conditions, report changes, and collaborate with the care team to adjust plans
• Perform routine assessments and assist with activities of daily living with dignity and respect
• Support admissions, discharges, and transfers while ensuring smooth continuity of care
• Contribute to a positive, team-oriented environment by communicating clearly and professionally

Qualifications

• Current LPN license in Pennsylvania (or compact/reciprocal license where applicable)
• Certified in Basic Life Support (BLS) — or willing to obtain upon hire
• Comfortable administering meds, performing assessments, and documenting care
• Reliable, patient-focused, and able to work collaboratively with nursing staff and leadership
• Previous long-term care or skilled nursing experience is a plus but not required

Education

• Completion of an accredited Practical Nursing program
• High school diploma or GED required

Knowledge & Skills

• Strong medication administration and documentation skills
• Good clinical assessment and observation abilities
• Ability to prioritize tasks and manage time effectively during an 8-hour shift
• Clear verbal and written communication for handoffs and interdisciplinary collaboration
• Basic familiarity with electronic health record documentation (or willingness to learn quickly)
