Roles & Responsibilities

Assist the physical therapy team in delivering hands-on care and restorative programs to residents in the skilled nursing setting
Implement treatment plans developed by licensed physical therapists and document resident progress clearly and promptly
Provide therapeutic exercises, gait training, transfers, and mobility assistance to promote independence
Collaborate with nurses, therapists, and family members to create cohesive care plans and communicate changes in condition
Ensure safe use of equipment and promote fall prevention strategies during therapy sessions
Contribute to a positive, resident-centered atmosphere by encouraging participation and celebrating progress

Qualifications

Licensed PTA in Illinois or eligible for Illinois licensure (facility may accept those eligible to apply)
Open to all experience levels — any experience accepted, including New Grad
Current CPR/BLS certification if required by the facility (obtainable if needed)
Ability to pass background check and meet facility onboarding requirements
Strong interpersonal and documentation skills; comfortable working in a SNF environment

Knowledge & Skills

Foundational knowledge of therapeutic exercises, modalities, and gait training
Skilled Nursing Facility (SNF) experience or familiarity with long-term care workflows is a plus
Patient handling and safe transfer techniques
Clinical documentation and progress note proficiency
Teamwork and clear communication with interdisciplinary staff
