Roles & Responsibilities

• Provide compassionate, evidence-based physical therapy services under the supervision of the PT
• Assist in evaluating patient progress and implement treatment plans to improve mobility and independence
• Deliver hands-on interventions and therapeutic exercises tailored to each resident in the SNF setting
• Document treatments, progress, and outcomes accurately and timely in the medical record
• Collaborate with nursing, rehab staff, and families to ensure continuity of care
• Support discharge planning and transition of care with clear communication and patient education
• Maintain equipment safety and a clean, therapeutic environment for residents

Qualifications

• Licensed or certified Physical Therapist Assistant (PTA) in good standing (state licensure required)
• CPR/BLS current certification
• Comfortable working in a Skilled Nursing Facility (SNF) environment
• Strong communication and teamwork skills; compassionate bedside manner
• New grads welcome — any level of experience considered, including recent graduates

Education

• Graduate of an accredited Physical Therapist Assistant program (Associate degree preferred)

Knowledge & Skills

• Knowledge of therapeutic modalities, gait training, transfers, and ADL-focused interventions
• Skilled in patient assessment, documentation, and outcome measurement
• Ability to adapt treatment plans for older adults and medically complex patients
• Familiarity with infection control practices and SNF regulatory requirements
• Good time management and prioritization in a busy clinical setting
