Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
• Administer medications and treatments accurately, documenting per facility policy
• Assess and monitor resident condition and promptly report changes to the care team
• Collaborate with nurses, therapists and support staff to deliver individualized care plans
• Maintain clear, timely clinical documentation and contribute to shift handoffs
• Support infection control practices and ensure a safe environment for residents

Qualifications

• Active LPN license in Pennsylvania (required) — you must be legally able to practice in PA
• Completion of an accredited Practical Nursing program or equivalent vocational training
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfort with medication administration, basic wound care and ADL assistance
• Professional attitude, strong communication skills, and a team-first mindset; 1+ years of LPN experience preferred

Education

• Practical Nursing diploma or certificate from an accredited program
• High school diploma or GED

Knowledge & Skills

• Medication administration and accurate charting
• Basic wound care and restorative nursing practices
• Familiarity with electronic health records and documentation standards
• Ability to perform resident assessments and recognize clinical changes
• Strong interpersonal skills for working with residents and families
• Time management and prioritization in a busy skilled nursing environment
