Roles & Responsibilities

Provide compassionate direct patient care to residents, putting dignity and safety first
Administer medications and treatments as directed by licensed clinicians
Perform routine assessments and document patient status accurately
Collaborate with interdisciplinary team members to create and follow individualized care plans
Assist with activities of daily living and support patients’ comfort and mobility
Communicate clearly with patients, families, and staff to ensure coordinated care

Qualifications

Valid LPN license in New York (required)
Current Basic Life Support (BLS/CPR) certification as required by facility
Comfortable working 8-hour shifts and providing hands-on patient care
Strong communication skills and ability to work collaboratively in a team
Ability to pass facility-required background and health screenings

Education

Practical Nursing diploma or certificate (LPN program) or equivalent training

Knowledge & Skills

Solid knowledge of medication administration and monitoring
Skilled in basic wound care and patient assessment
Proficient documentation and charting practices
Strong infection control and safety awareness
Empathetic bedside manner and good interpersonal skills
