Roles & Responsibilities

• Administer medications and treatments safely and accurately to promote patient well-being
• Monitor and record patient vital signs, reporting changes promptly to the care team
• Provide compassionate bedside care and assist with activities of daily living
• Document patient care clearly and timely in the facility charting system
• Collaborate with nurses, therapists, and physicians to deliver individualized care plans
• Educate patients and families on care routines and discharge instructions with clarity and empathy
• Support a positive, respectful environment that prioritizes patient dignity and safety

Qualifications

• Active and unrestricted Licensed Practical Nurse (LPN) license for New York State
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Previous practical nursing experience in a clinical or long-term care setting is preferred
• Strong communication skills and the ability to work effectively within a multidisciplinary team
• Comfortable with basic clinical documentation and medication administration procedures

Education

• Graduate of an accredited practical nursing program (LPN diploma or certificate)

Knowledge & Skills

• Medication administration and safe documentation practices
• Basic assessment skills: vital signs, pain assessment, and patient observation
• Infection control standards and proper use of PPE
• Patient-centered care and strong interpersonal skills
• Ability to prioritize tasks and handle multiple patient needs during a shift
