Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across Tehama County
Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
Develop personalized treatment plans for orthopedic, joint replacement, and geriatric patients with multiple comorbidities
Complete approximately 5 meaningful patient visits each day using the HCHB EMR system
Collaborate with the intake RN and office manager to coordinate exceptional patient care
Travel throughout the service area while enjoying mileage reimbursement for your daily 40-mile average

Qualifications

Current Physical Therapy license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
Strong assessment and documentation skills
Ability to work independently while providing outstanding patient-centered care

Knowledge & Skills

Physical Therapy expertise in home health settings
Proficiency with HomeCare HomeBase EMR system
Experience with orthopedic and geriatric patient populations
Knowledge of CHF and COPD management
Excellent communication and time management abilities
