Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver personalized physical therapy services to patients recovering from surgeries, falls, and other conditions in their homes
Build meaningful connections with home health and hospice patients while helping them regain strength and independence
Collaborate with interdisciplinary teams to create care plans that truly make a difference
Document progress using HomeCare/HomeBase EMR so every member of the care team stays informed
Travel within a 60-mile radius of Valley City to bring expert care directly to those who need it most

Qualifications

Current North Dakota Physical Therapy License
BLS certification required
At least 1 year of previous home health experience
Valid driver's license and auto insurance
Strong PT clinical skills and a passion for helping patients in their own homes

Knowledge & Skills

Home health and hospice patient care expertise
Comfortable working independently without on-site manager supervision
Familiarity with HomeCare/HomeBase EMR
Excellent time management to handle 5-7 patients daily
Compassionate communication with patients and families
