Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across Yolo County
Assess and evaluate patients' therapeutic needs while developing personalized care plans
Guide patients and families through rehabilitation exercises and therapeutic appliance use
Collaborate with physicians to report progress and adjust treatment plans as needed
Train patients in prosthetic device use and supervise home health aides during rehabilitation
Document clinical progress and participate in case conferences with care teams

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong oral and written communication skills with excellent organization abilities
Valid Physical Therapy license for California practice

Knowledge & Skills

Home health physical therapy assessment and treatment planning
Patient and family education techniques
Documentation and clinical reporting proficiency
Understanding of Medicare and Medi-Cal regulations
Ability to work independently across multiple service territories
