Roles & Responsibilities

• Provide attentive, hands-on personal care to patients with dignity and respect
• Assist with activities of daily living such as bathing, dressing, and mobility
• Monitor patients and report any changes in condition to nursing staff promptly
• Document care provided accurately and timely in the patient record
• Collaborate with nurses and the care team to support individualized care plans
• Maintain a safe, clean environment and follow infection control protocols

Qualifications

• Current LNA certification (required)
• Active authorization to practice in New Hampshire (state registry or license as applicable)
• Current CPR/BLS certification (preferred or facility-required)
• Recent clinical experience in long-term care, acute care, or similar settings is a plus
• Clear background check and up-to-date immunizations as required by facility policy
• Compassionate attitude, strong communication skills, and ability to work as part of a team

Education

• High school diploma or equivalent
• Completion of an approved nursing assistant training program and successful state competency testing

Knowledge & Skills

• Basic patient care techniques and mobility assistance
• Observation and reporting of patient condition and vital signs
• Fundamental infection prevention and hand hygiene practices
• Clear, respectful communication with patients, families, and team members
• Basic documentation skills and following facility protocols
• Ability to follow care plans and prioritize patient safety
