Roles & Responsibilities

Provide compassionate direct patient care within your LPN scope of practice
Administer medications and treatments accurately and document thoroughly
Monitor patients’ conditions and communicate changes to the care team
Assist with daily living tasks while promoting patient dignity and comfort
Support admissions, discharges, and transfers with professionalism
Maintain clear, timely charting and follow facility policies and procedures

Qualifications

Current, active LPN license (must be valid in Pennsylvania)
Completion of an accredited practical nursing program
Basic Life Support (BLS/CPR) certification or willingness to obtain
Ability to pass background check and meet facility immunization requirements
Strong communication skills and a patient-first attitude

Education

Completion of an accredited LPN/Vocational Nursing program
High school diploma or equivalent

Knowledge & Skills

Solid knowledge of medication administration and basic wound care
Comfortable with vital signs monitoring and patient assessments
Familiarity with electronic medical records and accurate charting
Good clinical judgment and the ability to escalate concerns appropriately
Team-oriented mindset and strong interpersonal skills
