Candidate Pay Rate: $56 per hour

Roles & Responsibilities

• Conduct comprehensive patient assessments and develop individualized treatment plans
• Deliver skilled physical therapy interventions in home settings to improve mobility and independence
• Document patient progress accurately and promptly in the charting system (Epic preferred)
• Collaborate with interdisciplinary team members and caregivers to optimize care plans
• Perform home safety evaluations and recommend adaptive equipment or modifications
• Participate in Trinity specialty competency testing and skills checklists to maintain high standards

Qualifications

• Current, active Licensed Physical Therapist (state license verification required)
• BLS certification (required)
• Home health physical therapy experience preferred
• Ability to complete required personal health screenings and immunizations (MMR, Hep B, Varicella, Tdap, COVID-19, Influenza, PPD/IGRA, 10-panel drug screen)
• Willingness to complete Trinity-specific competency testing and skills checklist

Knowledge & Skills

• Proficient in patient assessment, therapeutic exercise, gait training, and mobility assistance
• Comfortable with home environment evaluations and adaptive equipment recommendations
• Strong documentation skills; Epic experience preferred but not required
• Excellent communication and caregiver training abilities
• Ability to work independently in community/home settings and manage caseload
