Roles & Responsibilities

• Evaluate and treat patients across the lifespan in Outpatient, Acute, and Swing settings using evidence-based PT interventions
• Develop individualized treatment plans and goals that restore function and improve quality of life
• Provide occasional consultation and treatment for Long Term Care residents when requested
• Document timely, thorough clinical notes and communicate patient status with the care team
• Collaborate closely with the on-site OT and multidisciplinary staff to ensure coordinated patient care
• Educate patients and families on home programs, safety, and injury prevention
• Maintain current BLS certification and adhere to facility policies and standards

Qualifications

• Alaska state license to practice as a Physical Therapist (required)
• Graduate of an accredited Physical Therapy program (BS, MA or MS in Physical Therapy)
• Minimum two (2) years of direct patient care experience across varied settings
• Current BLS certification (required)
• Professional, friendly, adaptable demeanor with excellent verbal and written communication
• Able to work collaboratively as part of a team and shift focus as priorities change
• Basic computing skills (email, word processing, spreadsheets) for documentation and communication

Education

• BS, MA, or MS in Physical Therapy from an accredited program

Knowledge & Skills

• Hands-on patient care across Outpatient, Acute, Swing settings
• Skilled in assessment, treatment planning, and therapeutic modalities
• Strong verbal and written communication for patient education and interdisciplinary handoffs
• Comfortable with basic computer tools for documentation and scheduling
• Teamwork and flexibility in a dynamic clinical environment
• Awareness of long-term care needs and ability to consult/treat LTC residents when needed
