Roles & Responsibilities

• Treat patients as directed by the supervising Physical Therapist, delivering compassionate, effective care
• Record daily treatment notes and weekly progress notes in accordance with PT Board requirements
• Assist in maintaining the department and ensure treatment areas are clean and equipment is functioning
• Participate in patient care and rehabilitation conferences to coordinate interdisciplinary care plans
• Instruct patients, families, and nursing staff in maintenance programs and caregiver training for discharge readiness
• Report equipment issues promptly and support upkeep to ensure safe, reliable therapy delivery
• Communicate patient progress, problems, and plans clearly with supervisors and care team members
• Support in-services and training programs for facility staff, including restorative aide training

Qualifications

• Licensed Physical Therapist Assistant in good standing with the appropriate state board (California license required for this role)
• Experience providing direct patient care in a rehabilitation or post-acute setting
• Proficient with electronic documentation systems such as Casamba and PointClickCare
• Strong interpersonal and communication skills to collaborate with patients, families, nursing and therapy teams
• Able to follow physician and therapist treatment plans and document treatment changes per policy
• Willingness to participate in continuing education to meet state requirements

Education

• Licensed Physical Therapist Assistant (state license required)
• Must meet continuing education requirements per state practice (maintain license in good standing)

Knowledge & Skills

• Direct patient care and rehabilitation techniques
• Treatment documentation and weekly progress note best practices
• Care team communication and interdisciplinary collaboration
• Patient/family instruction and caregiver training strategies
• Familiarity with Casamba and PointClickCare or similar EHR/therapy platforms
• Basic equipment maintenance and safe handling of therapy tools
• Comfort working in a busy post-acute/rehab environment and contributing to team-based care
